I have friends who work in a restaurant who asked me what to do for stains caused by milkshake spills on their t-shirts or uniforms. They had tried all my suggestions pre-treat with Oxi-clean, lemon juice, dish detergent, ginger ale, GoJo, Spray and Wash, peroxide, and bleach on whites. Does any one have another suggestion that could work? Since this is a protein stain, do you treat it differently?

I have a friend who is a nurse. She swears by Zout for blood stains on her uniform. Since that too is a protein stain I thought it might help in this situation.
Also, if you've tried dish detergent, I'd be sure to try Dawn which is supposed to be better for grease. These would be fat stains on the clothing because of the fat in the milk or cream, imho. Best of luck
I forgot to say that I use cold water for stains, but in this case because it is a greasy stain, I'd try hot water the next time I washed it.
Try white toothpaste. Smear some on the stain before washing, let set for a while, then wash. See if that doesn't help.
Boil in dish washer detergent on top of stove. The granular kind like Cascade. Takes stains out of baby clothes even. Clothing must be of sturdy material. Couple spoonfuls in pot of appropriate size. Check stain every few minutes. Rinse well then wash in your clothes washer.
I don't know about milk shake stains but putting vinegar in the rinse cycle does remove stains. Do not pour vinegar directly on clothes, just add 1/2 cup to the rinse cycle.
