Repairing Jeans Pockets

I have some jeans that are well worn and the back pockets are starting to loosen from the jean or starting to make a hole. How can I fix this?

Thanks for the iron on idea! I knew there was something I could do but couldn't put it together in my mind. As far as the other suggestion, if I had money I would. But no work = no money!

Time for new jeans!

I have used iron on patches for this problem. Put it on the inside then, with needle and thread, sew the pocket down where it has ripped away from the jeans.
