Wording for a Donation in a Person's Honor

I would like some help with wording for a donation in a person's honor.

Keep it simple. If the person is deceased write, "In Memory Of" or "In Remembrance". If the person is still living write, "In Honor Of" or "In Fondness". If the person died of or has some disease write,"In Hope Of A Cure".
