Blend cocoa and sugar in a small bowl. In a medium pan, heat milk to scalding. Mix about 1/3 cup of the hot milk into the cocoa and sugar and make a thick syrup, then pour syrupy mixture into the hot milk in the pan; stir until well blended. Add vanilla. Add marshmallows if desired.
