If it was a drawing/painting put it in a frame and sit it on top with some flower pedals. I think that really symbolizes what your marriage should be like you should always be there to take care of each other. Like the plastic ones or metal it doesn't matter. Go to your florist have her make cake toppers with them. Using 5 of them (hoping you have a more then one tier) them put something that has to do with each person in to it. For example your a nurse (the little flower fill this and with a piece of ribbion tired around it or what ever you want to use a tiny little play needle. When I think of nurse I think of needles. and for th fire fighter use a toy, and the children something that they love the most. it doesn't have to be there toys but something the same as). You end up with your cake looking elegant as well as very meaningful to everyone involved. You could then place some fresh or silk flowers or curled ribbon around the base or silk ivey depending on their colors or theme. I am going to do this for our wedding in October of this year- but we are using a tuscan/italian theme and I have some glass grapes to place on the top around the photograph(black and white) and silk grapevines around the base. Small mirror. glass figurines to somewhat resemble nurse,fire fighter, kids. glue them in a pose on the mirror and put on top of cake. You can probably find Playschool people and use them. I think the compamy sells brides and grooms. We made an arrangement of silk roses and babys breath, and attached it to a small margarine tub lid. It fit perfectly on the top tier of their cake.
