How about something to celebrate pain free...like people dancing on the flatbed--just be careful no one falls off because they are feeling too good (sorry, I could not resist the kinda lame humor.)

Have a medical symbol next to a marijuana leaf.
I'd decorate the whole float as a cloud and have cannabis shaped costumes for people on top. The cloud can represent comfort and ease from pain... and clouds of smoke!
Do signs that say pain free.Show someone in pain,then taking medical marijuana then doing their normal activity.Show person throwing away canes after using cannabis.
There are many different types of cannabis and many people have no idea about them. You could decorate the float with the different types of plants and tell their medical value and what the plants do for your health and recovery.
