Using Iron-on Transfers?

My iron-ons are falling off; what am I doing wrong? How can I make them permanent?

Iron on transfers

Step 1
Be sure your iron is very hot.

Step 2
Iron transfer.

Step 3
Iron item.

Step 4
Put items together and press hot iron on them.

Step 5
When cool, you can use clear paint or glue to outline the transfer to strengthen the hold.
