I just gave my 2 puppies a five in one shot. Now one is sluggish and has runny poop with blood and long white worms. Is this the meds working or something worse?

It is normal after de-worming for your dog to have blood and worms in their poop. It is just the worms passing through the intestines. As long as your dog is eating and playing, it will be okay.
Blood in the poop is not good. Diarrhea and sluggishness are symptoms of worms. I would call the vet about the blood in the stool.
Side effects of the 5 in one vaccination are usually tiredness and loss of appetite. With a puppy I would be suspicious that the diarrhea, worms and blood were from a dangerous load of worms, or Parvo or both. A heavy worm infestation or parvo can be fatal. You will need a Vet to determine what you are really dealing with. A heavy worm infestation needs to be dealt with by drugs only a Vet has available.
