If you want to make them yourself, you can go to:

to buy some templates for gift and other certificates.

They look fairly simple though and probably any printer could help you put your information into a form that would look good. They do give you the basic layout and information needed for a gift certificate.

Put gift certificate in the search box. They have several styles that you can order.
