Puppy Has Accidents in the House?

My 3 month old puppy has been with us for about 2 1/2 weeks and she only has accidents when we are home. We are vigilant on letting her out every 1-2 hours. However many times she pees at the door or she will sneak into the other room to relieve herself. Many times right before or after we let her out.

Normally I would say it's just that she cannot hold it, but when we are gone or when we are asleep (we put up a baby gate and she is either in the master bathroom or laundry room) she can hold it for 4-6 hours with zero accidents. I am not sure what to do to correct this issue!

Are there children in the house?

It seems to me the dog is over-stimulated. It is fine when the house is quiet and there isn't anything going on. It is not fine when people are awake. It is hard to see things from a dog's point of view. Think about these things:

Are there children? Do they leave the puppy alone? If no kids, do you talk to the puppy in a really excited tone of voice? Are you always picking him up? Do you treat him more like a human baby than a dog?
Is the TV always on? Is there ever really any quiet during the daytime?

Do you keep interrupting his naps to feed him, take him out or play with him? Is there any conflict in the house? Do people argue a lot? Dogs are very sensitive and respond to what's going on in the household, so be sure he isn't getting too stressed out.
