Problems Making White Chocolate Fudge

My white chocolate fudge always falls apart while trying to cut it and it's very grainy. I'm not sure if I am cooking it too long, my other fudges turn out great! Any Ideas?

I hope this link works, I just TIVO'd a show from Alton Brown about fudge (silly me, thought you ALWAYS made fudge with marshmallow creme, not that I've attempted either) and he mentioned crystals. Since there is no info. on the Food Network site, I found this.
