How can you make your own self rising flour; what are the amounts?

3 1/2 cups all purpose flour
1 3/4 teaspoons baking powder
1 3/4 teaspoons baking soda
1 3/4 teaspoons salt

For every cup of flour, add 1 teaspoon baking powder and 1/2 tsp. salt and blend thoroughly.
