Have you tried blank business cards? Print out from a program for making your own business cards in the style you want it. Then, once you've printed them out, use a single hole puncher. Put your twist tie in the hole and tie. This also works great for gift tags (if you have a program that also has a gift tag portion on it) except instead of twist tie, you can use curling ribbon. You can always use business cards and personalize them on the computer. You can make a page all the same or each different. Just leave room at one end to use a hole punch to put the ribbon through to tie on the package.
