House Training a Puppy?

I have a 4 month old Yorkie. She will go outside almost every time we take her out, which is about every 30/45 minutes. Before bed I can take her outside and she will do her business, but as soon as I put her in her crate to go to bed she will pee. She also has a few accidents in the house. At this point I don't know what to do to keep her from peeing in her crate. Any suggestions?
