Reviving Hot Fudge

I used to work at a sundae shop. We used to "revive" the hot fudge sauce that had set up, with hot coffee.

I used to work at a Dairy Queen. We'd keep the ice cream cone's waxy chocolate dip liquified by placing it in a large tin can (like a coffee can) in a water filled crockpot set on high at first and then lowered.

I once worked at a coffeeshop (a regular family restaurant) and we also added hot coffee to our canned hot fudge sauce, but it was added right from the start, so it wouldn't be quite as thick as normal. The coffee gave it a great flavor and the favorite item we prepared with it was 'hot fudge ice cream cake'.
Does anyone have a recipe for hot fudge sauce from scratch?

I found this recipe to be very delicious. Make sure you follow all the steps and use 1 cup of cocoa instead of the 1/3 cup, which is just not enough. It is much better with more cocoa as you will read in the reviews. Here is the link:
