Cat Pees in Litter Box But Poops on His Bed?

I have a 6 year old male cat that will pee in his litter box and for the last month won't poop in it. We have tried everything, changed the litter, changed the box, and added a second box with different litter. We thought he could be constipated, so we changed his diet and have been to the vet twice. When he pees and almost poops he runs out like he's scared and licks himself like it hurts. The last couple of times he digs on his bed and just poops like it's normal so I'm thinking there is something about the box he's scared of. Any ideas? We are at our wits end and it's not sanitary!

Did you change kitty litter brand recently then switch back to old litter. Something may have scared him while using the litter box.

Try moving the litter box. Make sure it has no smell to the litter. Cats don't like it.

Make sure the box is clean and has the right amount of cat litter.

Maybe, you could put down 2 boxes of litter in different locations around the house.

That is a tough one. It does sound like he had a painful experience in the litter box that he now associates with the box, whether it is still painful or not. If the Vet cleared him you might try another Vet just to be sure. Sometime cats can get a stricture that makes pooping painful. It can be helped by a savvy Vet. If you can find a cat only Vet with a good reputation that would be best. Then you need to get him to not associate his litter box with pain.

You said he poops happily on blankets, which tells me he probably is no longer in pain but still fearful. Could you set up a litterbox with torn newspaper instead of litter? Or maybe one of the paper litters that is not like what he is used to. If he accepts that you could gradually add the litter back a little at a time. Be sure to keep those boxes squeeky clean so he doesnt associate smells with the new boxes
